Telehealth Opportunity for NP Licensed in 35+ States

The Nurse Practitioner is a licensed clinical professional who performs clinical assessment of prospective patients and also provides oversight of RN Care Managers within various Client programs, like home-based Remote Patient Monitoring (RPM). This role acts within a multidisciplinary team and is supervised by a Medical Director. In addition to performing initial visits with prospective patients, the Nurse Practitioner also assists with resolution of patient escalations and interfaces with internal and external care team members, patients, and patient caregivers as appropriate.

Responsibilities:

  • Perform initial virtual, comprehensive assessments of patients referred to Client’s programs; for those who are clinically eligible, determine which devices are most appropriate for each patient.
  • Ensure documentation supports administrative functions such as quality management and billing.
  • Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of RN Care Managers and Medical Directors.
  • Respond to patient escalations identified within Client’s virtual programs, i.e. abnormal or outlier vitals, symptoms, or other complexities related to patient care.
  • Demonstrate a professional attitude, sound judgment, and empathy in all interactions with patients, caregivers, and other internal/external healthcare team members.
  • Be an integral part of a team of passionate healthcare experts spearheading the development of innovative, low risk/high benefit programs to keep patients healthy, happy, and out of the hospital!

Requirements:

  • Clinical Licensure:
  • Current and unobstructed NP licensure in Michigan
  • Ability/willingness to expand state licensure for new markets and partners
  • Education:
  • Graduate degree in nursing (MSN, DNP) or a post-graduate certificate in nursing field (for RNs)
  • Minimum of three to five (3-5) years of clinical experience, preferred in acute or ambulatory care
  • Experience working with the senior population in home health, clinics, or other health service setting (Preferred)
  • Experience thriving in a fully virtual environment (Preferred)

Knowledge and skills:

  • Skilled in nursing process
  • Excellent clinical acumen
  • Empathy, patience, and passion for delivering a phenomenal patient experience
  • Exceptional written, verbal and interpersonal communication skills
  • EHR/CMR experience
  • Highly personable, social telephone manner
  • Work effectively with minimum supervision
  • Work well in a team environment

Working Hours:

  • Commitment to a minimum of 20 hours per month of pre-scheduled shifts during Monday to Friday from 8:00 a.m. to 8:00 p.m local time.

Compensation:

  • Starting at $72 per hour
  • Training time will be compensated at an hourly rate
